If you contact Andrea, your contact details and message are used to respond and coordinate the services you request.

Online booking collects your contact information and home-visit address. Visit notes are for parking, gate access and setup. Payments and card details go through checkout and its payment provider. Keep health and card details out of ordinary messages.

After booking, use the intake link in your confirmation or reminder and the same email as your booking. It asks about health, goals and comfort, with an acknowledgment and signature. Answers and signatures are stored in restricted Wix records for Andrea to review before treatment. Access is limited to Andrea and authorized service providers supporting the service.

If online intake is not possible, call or text 386-895-CALM (2256) to arrange private completion with Andrea before treatment. Do not email or text health histories or completed forms. Downloadable PDFs are blank reference forms; downloading one does not submit intake.

Records are kept for care and applicable record-keeping requirements. To request access, correction or deletion, contact Andrea using the details below without including health information. Andrea verifies identity and reviews any retention obligations before acting. Deletion may be limited by those obligations.

Newsletter signup is optional and separate from booking and treatment consent. Wix uses your email and subscription choice to manage it. Contact Andrea to change your choice.
